What are the Three Types of Health Insurance?

Health Maintenance Organizations (HMOs)

Defined as being a local network of doctors, hospitals, and other healthcare specialists and facilities that participate in the health maintenance organization, you get to choose a primary care provider (PCP) in that organization. Your PCP has to refer you to see a specialist that is in-network.

Preferred Provider Organizations (PPOs)

Under a preferred provider organization, you have a whole gambit of providers, doctors, hospitals, and healthcare facilities to choose from. You don’t have to select a PCP under a PPO and you can see a specialist without a referral. You can also see providers out-of-network, but you will pay more out-of-pocket.

Exclusive Provider Organizations (EPOs)

Similar to an HMO, an exclusive provider organization allows you to choose from participating providers. Depending on the policy, you may or may not be required to have a PCP, and if you want to see an in-network specialist, you don’t need a referral. However, the costs of visiting an out-of-network facility or doctor are only covered in an emergency.
